i had a fan go out on me in my computer. not sure though if i should measure diagonal or straight across. it's a small fan fits in the palm of my hand and it sits over top a thick silver piece with lines and spaces on it through it. it also could fit in my palm.

It's measured across the edge, so a 3" fan would be 3 inches on a side.
It really doesn't matter how you measure it, though, as long as you take a ruler to the store and measure the new one the same way.
Eventually, you will have to take the old one off to replace it with the new one, so why not take the old one to the store with you, as the other poster suggests? That will also let you make sure that the leads are long enough, although replacement fans usually have wires that are way longer than you need, anyway.
Take it out and bring it with you to a computer parts store such as Fry's or even Best Buy (if you want to pay triple the amount for a stupid fan)
