I have a USB 2.0 card reader. I have just check to see which cards it can be used with. I can use it to read SD/MMC cards as well as MS, CF and X-memory. I just put the card in the correct slot and plug the reader into a USB port. The card is view by the computer's operating system as a disk. I use mine to transfer files and updates for my SatNav.
You either connect your phone to your computer trough a usb cable and use the software that is given with the phone to transfer files (example: samsung has some really nice software for that).
If you don't have software, but your computer has an sd slot build-in, just put the card in there and your computer will recognize it as an drive.
If you have a laptop or desktop that has bluetooth, try adding your phone as hardware on your computer trough bluetooth. You will be able to transfer files.
