Gantz fits the bill and it's pretty amazing to start. It kind of jumps the shark later on though.
Black Lagoon is a great manga(and anime) as well. It's not "smut" but it doesn't shy away from nudity at all.
Hellsing is over the top, violent, and awesome.
Vagabond is a modern classic, some of the most amazing artwork around. Check it out.
Vinland Saga is a brutal, detailed and intelligent story about Vikings.
Moving away from the overtly violent stuff, there are plenty of intelligent and dark stories that aren't as over the top.
Check out Naoki Urasawa, specifically his series Monster, but also Pluto, 20th Century Boys, and Billy Bat. All of them are very intelligent mystery/psychological thrillers.
If you like mind games, Nobuyuki Fukumoto is very famous for series that revolve around gambling. Akagi and Legend of Strongest Man Kurosawa are probably his two best, but most of his work is worth looking over.
Another series in the same vein as the above Author would be Liar Game, by Shinobu Kaitani. His other work of note, One Outs, is also excellent.
