It depends entirely on the doctor. Some will just talk with you about the various problems you've had and decide you have it. Others will do more formal work ups with diagnostic tests. In my own case, they sent me in for a full, day-long "neuropsych" evaluation, which was a full 9-5 day of every kind of test you can think of - memory, cognition, attention span etc. I actually went in trying to figure out a math learning disability as an adult and was also diagnosed ADHD. That was in my final months of good insurance, so it cost me nothing, but probably would have been thousands out of pocket. Doing it on a small budget is tricky, I know firsthand. The immediate release generic drugs can be as little as $20 a month, but if they're controlled stimulants, the doc needs to see you every three months or so and that's $150 a pop, give or take. The fancy newer drugs are heinously expensive without insurance. On the other hand, if it means the difference between being able to hold a job and not, it sort of pays for itself in the long run.
