Your local post office or court house is the most conveinent and easiest. You will need an appointment at most of the post offices. Some offer to have your picture done there and some you have to bring your own. You can get them done at places like Walgreen's. It only takes a few minutes. The court house does not require appointment but has limited hours and you will need to take your photo with you. Take your birth certificate, and drivers license. You will need cash and or check. Total cost @ $150. They are good for 10 years. You need at least 4 weeks to get them back. They will tell you 4-6 weeks just depending on the mail. It took mine 4 weeks.
I am assuming you are a US citizen. While you can download and print the application forms on line, for an initial passport you MUST make an appointment to meet with passport representatives. You will need your application, a certified copy of your birth certificate, proof of name change if the name you use is different from the one on your birth certificate, and two photos that meet the passport office requirements. And money. You can find the exact requirements, fees, and procedures at the US State Department web site, which is very easy to negotiate and very helpful. It also has the forms, and is at http://www.state.gov./
If you have a trip planned within the next two or three months, get on this right away! You may need to pay extra for expedited service. Also, if you don't currently have a certified copy of your birth certificate, apply for that immediately! Depending on where you were born it can take anywhere from several days to weeks to get. You can get the photos taken at just about any Walgreen's or other chain pharmacy and many of the "big box" stores for about $15.
