Muv Luv/Muv Luv Alternative - Muv Luv is a fairly generic title, however it sets up Muv Luv Alternative, which is generally considered to be among the best visual novels available in English. Alternative (and the second half of the original) are more mecha action though rather then pure romance like Katawa Shoujo

Clannad - Key Romance title, one of the absolute best romance heavy titles in English (or otherwise)

Yume Miru Kusuri - Not quite as good as the above, however this is probably the closest translated game to Katawa Shoujo.

Fate/Stay Night/Tsukihime - You may know the anime for these Type Moon works, the original Visual Novels however are vastly superior to their anime.

G-Senjou/Sharin no Kuni - 2 highly rated titles by Akabesoft romance series with action/mystery elements

Umineko no Naku Koro ni/ Higurashi no Naku Koro ni - You may know these anime as well, these are the two series by 07th expansion, which are more mystery/horror.

Maji de Watashi ni Koishinasai - 3 of 5 routes translated, 4th route should be out shortly, very fun romance title

Symphonic Rain - An often overlook, but very enjoyable romance with some great music

KiraKira - A guy and 3 girls put together an all girls band, hilarity ensues.

Cross Channel - A highly rated repeating romance/mystery with some outstanding heroines.

I've never played Katawa Shoujo so I cannot recommend something similar, but, I think you should REALLY check out Higurashi No Naku Koro Ni, and Higurashi No Naku Koro Ni kai. They are a bit old so you might take a while to find them, but god is it worth it, there is also an anime based on this visual novel if you would prefer that intead.

it depends on how much against "adult material" you are. as noted before, Higurashi no naku koro ni is a good one, though it is old and not of the highest art quality... there are also a few like Analog: A Hate Story which is available on Steam. (the two i just mentioned do not have any sexual material) if you want a nice story, there are plenty of eroge(visual novels with sexual content) out there that actually have good stories. Utawarerumono, Wanko to kurasou, Fate/Stay Night(its LONG and text heavy), Kira★Kira, Family Project. those are a few examples. just remember, when you dive into eroge, you will find more porn than content usually, as most are like reading 50 shades of gray for men... its uncommon to find an eroge with a good story, but once you find one with a good story, its one hell of an awesome ride.
