It depends on who you are insured with. In some cases, insurers will give you third party only cover while you are driving a vehicle which does not belong to you and is not hired to you under a hire purchase agreement. However, there does also need to be insurance on the car itself, so the owner of the car needs to take out a policy to cover it. The extension is just there in case you need to drive the car and are not a named driver under the owner's policy.
One thing to bear in mind though is that you are only getting third party only cover under this extension, regardless of the cover you have on your own car, so if you have an accident, the insurer will pay out for damage to anyone else's car or property if you are to blame, but you will have to foot the bill for damage to the car you are driving under the extension yourself.
Some insurers have now withdrawn this cover, so I would check your certificate of insurance or speak to your insurer before driving the car, just to make sure the cover is in place.
In the USA, with rare exceptions, any time you drive an insured car with the owners permission and have a valid drivers license, you will be covered by whatever insurance has been purchased for that car.........but....... You and you G/F almost certainly have more coverage than just comprehensive? Comprehensive only coverage is normally used when a car is parked, in a garage and not driven. You cannot legally be on the road with a car that has only comprehensive coverage. That aside, you need to sit down with your agent and allow him/her to educate you about auto. insurance so you don't get in trouble.
Almost always if the car is insured anyone who drives it is insured but if you live in the same household you should be named on the others policy as an additional driver. If not although the insurance company would almost always pay for damages if one of you had an accident in the others car they might cancel the insurance or charge a ridiculous premium. I can't do better than usually or almost because there are policies which have restrictions on underage drivers.
My advice, do the smart thing and have yourselves added as additional drivers if you're in the same household.
Generally the insurance on the car also covers others driving on an occasional basis with the owners permission. Check the policy to be sure because some do not allow it, or only allow those over age 25 to be covered.
If you are boy and girlfriend, it sounds like it's more than occasional basis though, and to be sure you should be added to the others policy as an additional driver,especially if you share an address.

It will tell you on your car insurance, or a quick call to them and they will tell you. With some companies you have to be 25, with others you have to be a named driver, they are all different. Make the call and put your mind at ease :)
