Two Claim addresses on insurance card?
I need information for the claim address on my insurance card but there are two different addresses: one for medical claims and another for pharmacy claims. Which one should i choose? Thanks in advance.

?? If you are submitting a claim for a doctor or hospital visit, you use the medical claim. If you're submitting a claim for prescription drug benefits, you use the pharmacy claims.
The service provider files all claims. You don't use either address for any reason ever
