How are tax policy enforcements different in North America than that in India for private doctors or hospitals.
These are people who charge huge amounts of money in minor treatments and exorbitant amounts in operations/surgeries, but do they contribute to the national treasury ?

In the U.S., doctors do pay tax on the money that they receive for doing their work.
However, the net effect is not a positive contribution to the national treasury. The government spends more on paying for medicaid, medicare, and health insurance for its own employees, than it receives from the taxes paid by doctors and hospitals.
Most hospitals in the U.S. are run by charities or the government, and do not pay tax. A few hospitals in the U.S. are regular businesses that try to make a profit, and they do pay tax.
I do not know what the situation is in India or Canada.
I think he was bitter about something else and decided to unload on the nearest person, regardless of occupation. It's an ignorant thing to say, and the truth of the matter is that with the population the way it is, doctors earn every penny, most of which will, for the first decade or so, go towards paying off the expensive education that allows them to keep ungrateful folks like him alive. I'm pretty sure doctors who do not work in public hospitals pay taxes. Not sure about those who do work in hospitals.

India has a huge rate on tax evasion, which should include a number of doctors.
