I have a contract with a company (don't worry, I've thoroughly looked it over) that I have to sign and fax back their way, but the signature section looks different from anything I've seen before. I'm avoiding asking them because I don't want to appear unprofessional, which is why this was my first resort. If you fail me I'm going to have to contact the company themselves, I've already asked my family and mentors!
The end of the contract looks like this:
ACCEPTED AND AGREED:
_______________________ [company name here], INC.
By: _________________________ By: _____________________________
I feel like I need to sign one of the "By:" lines and write something under the "accepted and agreed". Someone help, I am clueless. THANK YOU.

They are expecting you to sign as an agent of your company. Put your company name where directed, and sign underneath. If you have a second agent of your company sign, they sign in the second by line.
If you do not have a company and the contract is with you individually, then put your name in the first spot, cross out the Inc, and sign by the first By.
If you are a company put your company name in the first blank and sign one of the "By" lines.
If you aren't a company just sign one of the "By" lines.
